Select the purpose of use under the Secondary Use Act that your data request relates to. The application as a whole must match the purpose of use you select.

Describe what statistical data you are requesting and why. Describe your data request in enough detail that it can be carried out based on the application and its attachments alone. If needed, you can provide this information in a separate attachment.
Explain how the content of the statistics and the variables used in them match the purpose of use and meet your information needs. Also state whether you need a one-off or a recurring extraction.
